“The waiver granted by the commissioner or his designee shall be for a period not to exceed 2 years; except in any case where a licensed physician, advanced practice registered nurse licensed under RSA 326-B, or physician associate licensed under RSA 328-D documents a lasting medical necessity, in which case the waiver shall be for a period not to exceed 4 years.”

“A physician licensed under RSA 329, or a physician exempted under RSA 329:21, III, an advanced practice registered nurse licensed under RSA 326-B, or a physician associate licensed under RSA 328-D certifies that immunization against a particular disease may be detrimental to the child's health.”
